This looks like an adjustable Rolex case back remover.

Spin the thumb wheel until the little toothed pieces are the correct distance apart, then apply them to the caseback and twist

A rolex style caseback tool, not efficient though... I have a similar one and plan to get the rolex T-bar caseback set.
