Have you ever been 100% happy with a rep OTB?

Yes ... my Ebel 1911 Discovery Chrono was perfect OTB when it arrived. I did get it serviced, but no mods are necessary.

I know many have updated to gen dials for a BCE, but I've never seen the need to mod my v3.

If you got the black one it needs the AR done. The purple AR OTB is terrible with the black dial. (note i think it's not as big a problem with the white dial) I sent mine to Taka and the watch is stunning with his AR.

I have been as satisfied as is possible with this replica (at least the closed caseback is quite accurate) but thats just because I know that i will never ever run into a  FRANCK MULLER expert

Yes, but only a few times and only with quartz powered watches.

How come?

Because I figure mechanical movements are usually 'Dry and Dirty' so I dig into them. They turn out to be 'D&D' most of the time.

(I do not worry about 'D&D' quartz watches)

This shows what can happen with 'D&D' mech movements:

I'm wearing a rep right now which I have to say has been one of the best reps out of the box I have purchased. Its a 500m Calibre 5 Tag Aquaracer. Love it, and the Asian ETA clone is running spot on too.

As to other reps in my collection, one has been frankened, another is my TC sub waiting on insert, and another, a TT Sub C from Andrew. The Sub C will be worked on soon as this has some flaws that I can correct. The 12 oclock indice, and the one @9 too.

500m Tag, cannot agree more! The fact that it is still the only rep I have wear while swimming makes it even more satisfying. Unfortunately it is gone now.
